 Dr. Nerviano is a  provider for all major insurance, HMO, and EAP programs locally,
including , State and Federal employees, but not Medicaid  or Medicare.
I can take Medicare Advantage as it goes through regular insurers!! 
 
Psychological Testing (96101) may be an insurance benefit but may have a deductible, co-pay and/or additional paperwork approvals that do not apply to other mental health services.
(Court ordered evaluations, lethal weapons clearance, and most educational testing are not usually covered.)

The only way to know your current insurance or EAP coverage is to call them!  For insurance check your "outpatient mental health benefits" coverage. Check if you need prior authorization, have a deductible, co-pay or co-insurance, and when the coverage year ends!
(really, you need to call them and not depend on your card, a booklet or web site!!)

   There must be one identified client for contract, record and billing, even if it is an
EAP benefit for marital issues, or, family / couple counseling under insurance
(EAP is not 'medically necessary' so can cover any services: insurance is specific)

 These restrictions do not apply to self pay clients.
